Virginia Mason Franciscan Health recommended schedule based on CDC guidelines

Age	Recommended Vaccine
Birth	HepB in hospital
2 months	DTaP, IPV, HepB, Hib, PCV13, Rotavirus
4 months	DTaP, IPV, [HepB*], Hib, PCV13, Rotavirus
6 months	DTaP, IPV, HepB, Hib, PCV13, Rotavirus
6 months through adulthood	Influenza (yearly, fall/winter)
12 months	MMR, PCV13, HepA
15 months	DTaP, Hib, Varicella (chicken pox)
18-24 months	HepA
4 years	DTaP, IPV, MMR, Varicella
11 years	HPV (series of 2 doses over 6-12 months), Tdap, Meningococcal
16-18 years	Meningococcal

Many of these vaccines can be given using combination products, reducing the number of shots required. Examples include:

Pentacel = DTaP + IPV + Hib
Pediarix = DTaP + IPV + HepB
Kinrix = DTaP + IPV (for age 4 through 6)
ProQuad = MMR + Varicella

*Need for HepB at 4 months depends on combination products used
