Improving the Patient Experience

Our Care Management staff and physicians will work closely with you as you choose a skilled nursing facility (often called a nursing home), home health agency, or other care setting. We support our patients beyond our hospital walls through our Continuing Care Network (CCN):

• We work with medical providers in the skilled nursing facilities to provide oversight for patients with complex medical needs
• We have arranged for physicians and nurses to oversee the quality of care and patient satisfaction in our CCN
• We strive to improve the experience for patients and their families, and reduce unnecessary readmissions to the hospital

Partners in our network have undergone rigorous site review and many meet or exceed Washington state medians for federal quality standards and state patient satisfaction ratings.
